Proposal Form

Your proposal form will normally be sent to you from your insurance adviser. You may receive a statement of fact which needs to be checked to ensure that the statements given by you and recorded on the proposal form are in fact correct. Any changes must be notified to your insurance adviser.

General Notes on proposal form completion:

We suggest you keep a record of your proposal including copies of letters, for your future reference of any additional information given for the purposes of entering into the contract. A copy of the completed proposal form is available on request.

Data Sharing:

Please note that Insurers maintain a Motor Insurance Anti Fraud and Theft Register and a Claims and Underwriting Exchange Register. Information is exchanged with each other to prevent fraudulent claims. These services are run by Insurance Database Services Ltd (IDS Ltd) and the Association of British Insurers (ABI) . Your insurance cover details will be added to the Motor Insurance Database, run by the Motor Insurers’ Information Centre (MIIC). This helps identify uninsured riders, and may be searched by the police to help confirm who is insured to ride. If there is an accident, the Database may be used by insurers, MIIC and the Motor Insurers Bureau to identify relevant policy information.

Disclosure

You are not required to disclose convictions regarded as ‘spent’ under the Rehabilitation of Offenders Act, 1974 . It is an offence under the Road Traffic Act to make any false statement or withhold any material information. Any other facts known to you which are likely to affect acceptance or assessment of the risks proposed for insurance must be disclosed. Should you have any doubt about what you should disclose, do not hesitate to tell us or your insurance broker or adviser. Making sure we are informed is for your protection, as failure to disclose may mean that your policy will not provide you with the cover you require or may perhaps invalidate the policy altogether. Before signing the form, please read again the questions and answers, especially if not completed in your own hand.
